At Michael E. Miller DC - Wellness Huddle, we are committed to protecting your health information in accordance with the HIPAA Privacy Rule and applicable federal and state laws.
We are required by law to:

  • Maintain the privacy of your Protected Health Information (PHI)

  • Provide you with this notice of our legal duties and privacy practices

  • Follow the terms of this notice

1. Treatment - We may use your information to provide, coordinate, or manage your care.
           Example: Sharing information with labs, other providers, or wellness practitioners.
2. Payment - We may use your information to bill and collect payment.
           Example: Sending claims or superbills to insurance or processing self-pay documentation.

           Example: Staff training, quality improvement, compliance monitoring.

Reproductive Health Privacy Protections
We will not disclose reproductive health information for investigations or legal actions without:

  • Your authorization, OR

  • A valid legal requirement with specific attestation

Patient Access Rights
You have the right to:

  • Receive copies of your records quickly and in electronic format

  • Direct us to send the records to a third party

  • Receive an estimate of costs (if applicable)


👤 Your Rights Regarding Your Health Information

You have the right to:

Access Your Records:  Request Copies of your health records (electronic or paper)

Request Corrections:  Ask us to correct inaccurate or incomplete information

Request Restrictions:  Limit what we share (may not always be possible)

Confidential Communications:  Request we contact you in a specific way (phone, email, etc.) 

Accounting of Disclosures:  Request a list of disclosures made

File a Complaint: You may file a complaint without fear of retaliation

🔐 Our Responsibilities

We are required to:

  • Notify you if a breach of your information occurs

  • Protect your data using administrative, physical, and technical safeguards

  • Limit use/disclosure to the minimum necessary

✍️ Uses Requiring Your Authorization

We will obtain your written authorization for:

  • Marketing purposes

  • Sale of your information

  • Most sharing of psychotherapy notes (if applicable)

You may revoke authorization at any time in writing.
